Eniva Rewards Program Guidelines, Limitations, Terms and Conditions
1. No participation fee or purchase is necessary to enroll in Eniva Rewards.
2. Only secure online checkouts with a registered account are eligible to earn Rewards Points. An email address is required for a customer to receive Eniva Rewards communications, including coupons and offers. A customer is responsible for maintaining an accurate email address at all times; Eniva is not responsible for loss of Points or Rewards due to inaccurate customer information or email address.
3. Points can be redeemed for Store Credit. Note: Subscribe & Save customers will need to log into their account and apply their Store Credit to their order before their chosen auto-ship date. Points never expire, so you will keep accumulating Points until you choose to redeem & apply. Store Credit can be used in addition to one Coupon per order.
4. Customer must be 16 years of age or older to participate in the Rewards Program.
5. Tax, shipping, and Coupon redemptions are not included in the calculation of points earned from a purchase. Customers are responsible for all taxes where applicable.
6. Coupons must be used at time of purchase. Coupon value cannot exceed the value of the item. Cash back is not provided.
7. Coupons and Store Credit cannot be exchanged for cash or Gift Certificates. Rewards are valid for 180 days after issue or redeem date. Free Product Rewards and Store Credit will not be accepted or available after 180 days. You are responsible for using your Free Product Reward or applying Store Credit to your order within 180 days. Pro-tip: Redeem and apply same day!
9. If providing a testimony: By providing your testimony, you agree Eniva may use it in the promotion of its products through various means, and owns rights, title and interest to the contents of the testimonial. These rights include the right and permission for Eniva to use, re-use, publish, republish, reproduce and distribute to the public the Testimonial and use of your name; including copies and derivative works in which the original Testimonial is edited or adapted; made through any medium, and in any media now or later, including print media, radio, television and the Internet, for the purposes of illustration, promotion, art, advertising, or trade. You agree you do not need to inspect the final version used. Eniva is not liable for any results of blurring, or distortion or alteration that may occur. Eniva reserves the right to substitute a picture of similar or related content, as well as verbiage used throughout, per the meaning and intent of the testimony, and to meet regulatory and compliance requirements. You agree exempt from any claims of libel, defamation or violation of rights of privacy or publicity. You agree you are of legal age (and any children in videos are included in your permissions for Eniva Health's use).
10. Eniva has the right to limit Referral and Share Points.
15. Shipping and handling costs resulting from online transactions are not eligible to be offset with Points. Some special exceptions of limited-time-offers may be allowed. If a Customer believes his or her eligible purchase was incorrectly applied toward earning Points, they should contact Eniva at 866-999-9191. Requests for an adjustment must be received by Customer Service within 60 days from the date of activity. Written requests should include the customer’s name, email address, street address and daytime telephone number. Failure to supply the adequate and authentic documentation may result in denial of adjustments.
16. Repeated return of merchandise as part of this Rewards Program may result in termination of Customers’ Rewards account.
17. Continued use of the Eniva Rewards program constitutes acceptance of any amendments to the Program.
18. We reserve the right to change or discontinue the Rewards Program, or a membership in the Rewards Program at any time, without notice. If the Program, or a membership is cancelled by either party, all Points are forfeited.

20. Customer releases Eniva and affiliated parties, and their respective officers, directors, employees and agents from and against any and all liability relating to that Customer’s Eniva Rewards participation or this agreement.
21. This Agreement shall be governed by and construed under the substantive laws of the state of Minnesota, without reference to conflict-of-laws.
